*FYI: Did you know your medical records request fee can be waived? Only when your medical records are sent from provider to provider.
*Please bring the following documents for a brief overview of your claim/s.
Be prepared there's a fee if you personally request for your medical records. The medical records fee cannot be waived as it takes a process to be gathered by a human, ink, and paper to produce for you. Please do not argue or demand with your provider HIM / Medical Records department as you are not the only client that is requesting documents, please be proactive. Providers are required to provide medical records within a time frame - perhaps smile or bring them store bought cookies as a token of appreciation!
